Ten-year-old computer glitch prevents delivery of 1,380 Canadian health results

Sunday, May 24, 2009

The Saskatoon Health Region in Saskatchewan, Canada recently discovered a fax machine problem which had not relayed almost 1,500 X-rays, Computed tomography (CT) and Magnetic resonance imaging (MRI) scans, ultrasounds, and other medical imaging test results to doctors.

The fax machine is part of the automated Radiology Information System. An audit revealed that medical diagnostic tests involving results dating back a decade have been affected by the broken fax machine.

On May 6, a doctor called the health region officials seeking follow up on a patient’s report. Staff who delved into it, found that the report was not sent from the system.

Now, two weeks later, this same patient has still not been contacted by the health region.

“It has taken some time to work with the patient’s physician. That was important — to get more information and more detail because we want that patient’s physician to be involved in the discussion with the patient,” Dr. David Poulin, vice-president of medical affairs for The Saskatoon Health Region, said. “This is a system error and that’s just what it was — an error. This doesn’t reflect in any way on the quality of work regional staff have done and continue to do.”

An internal review revealed that, of 2.2 million diagnostic tests performed, at least 1,380 had not been sent out by the malfunctioning fax machine.

The health region will embark on contacting each of the 1,380 patients and their physicians to make sure that the results have been received and if any health care was compromised.

Patients can also contact the health region via a newly set up hotline to make enquiries.

“We think it would actually be good practice if physician offices could have a system to check whether they have received important reports,” said Poulin, “The common practice appears to be in many doctors’ offices, particularly family physician offices, that they don’t respond to the report until it arrives. So, they basically are waiting for the report to arrive in their office by fax and it’s at that point that they look at it and decide what action to take.”

City Hospital, St. Paul’s Hospital and Royal University Hospital were the three city hospitals affected. Saskatoon has a population of roughly 233,923, and the hospitals also serve the surrounding rural areas.

Ironically the faulty fax machine was discovered the same week that provincial medical officials began to review approximately 70,000 radiology tests conducted in Yorkton. Officials there doubt the competence of the physician who first interpreted the radiology results.

IMF and EU approve aid for Georgia

Tuesday, September 16, 2008

The International Monetary Fund and the European Union approved aid packages to help Georgia recover from its conflict with Russia, which occurred in early August. The IMF approved a US$750 million loan which will allow Georgia to rebuild its currency reserves. The European Union also approved an aid package of 500 million in aid by 2010, which is expected to help internally displaced people (IDPs) and economic recovery in the form of new infrastructure. Only €100 million of the EU aid will be given to Georgia this year.

These loans are aimed to restore confidence in Georgia’s economy and send a signal to international investors that Georgia’s economy is sound. According to the IMF, international investors have been “critical to Georgia’s economic growth in recent years.”

Takatoshi Kato, Deputy Managing Director and Acting Chairman of the IMF executive committee, said the loan will “make significant resources available to replenish international reserves and bolster investor confidence, with the aim of sustaining private capital inflows that have been critical to Georgia’s economic growth in recent years.”

Georgia has requested $2 billion in international aid to help it recover from the conflict. So far, the United States has pledged $1 billion in aid. Further assistance and loans to Georgia are expected from other organizations. Kato noted that “…Georgia is expected to receive financial assistance from multilateral and bilateral donors and creditors in support of the reconstruction effort.” It is expected that an international donors’ conference will take place next month to solicit more aid for the country.

Georgia’s government expects that economic growth will be more than cut in half as a result of the conflict. Last year, Georgia’s GDP increased 12.4% and it is predicted by the IMF that growth will be less than 4 percent in the coming year.

Ways To Win More In Lottery

Submitted by: Nicholas Jarder

The chances of winning lottery especially the jackpot would usually change depending on the type of game design and the factors that constitute to the numbers. Some of the factors include the use of possible number schemes such as the winning numbers drawn and the type of game applied. For example, the game 6-from-49 lotto allows players to pick at least 6 numbers which are from 1 to 49. The players can pick 6 digits from that combination yet they are not allowed to pick the same numbers. If the numbers which are picked by the player is shown in the official drawing then the player would be able to win the jackpot. There was once a mathematical equation used to decipher the chances of winning in jackpots. The results of the equation has shown that winning can be 1 in every 13,983,816. This means that winning is fairly hard especially when you are planning to take home the jackpot. However, the odds of winning are increased with the use of best pick 3 lottery software and with the right game.

Best pick 3 lottery software is a type of program that allows users to get several combinations that are computed with the use of a specific algorithm. The algorithm is actually based on previous number combinations that are recorded within the program. Since the computation for the combinations are picked by a program, the chances of winning would significantly be increased. In terms of game, winning can also be achieved through picking the right type of lottery. For instance, in bonus ball lottery, the bonus ball used is quite compulsory and the odds are much higher since there is an extra ball added in the game. In terms of the multistate type of lottery, numbers are drawn in 56 to 1 and are drawn in groups of 46. The player would need to match all the six balls in order to win. Because of the extravagant amount of combinations, winning would be slimmer so in essence, bonus ball lottery is better quite better than the multistate lottery. The hardest type of lottery to win is the SuperEnalotto which is in Italy. The players would need to match 6 numbers which is rounded of in 90 meaning the chances of winning is around 622,614,630 to 1.

Due to the differences in winning, it is important for players who would want to win more to choose the right type of game. For instance, in real life lottery is quite different from computer based games because the numbers are not as random. So, chances of winning would be better. The number of players would also have a significant effect on the payout. If more people would join, the chances of splitting the winnings would extremely be high. It is quite better to bet if the expected value of bets would notably be low since although the price is affected the chances of winning are high. Using the right program such as the best pick 3 lottery software would also improve the chances in winning since such programs are known to use a specific pattern. So in essence, in choosing a lottery program, you would definitely increase your chances of winning.

In February 2017, the Iranian Chess Federation announced two teenage chess players, Dorsa Derakhshani and her younger brother Borna Derakhshani, were banned from representing the national team. The federation announced their decision although Dorsa Derakhshani had previously decided and informed the chess federation she did not wish to play for Iran.

Dorsa Derakhshani is currently 21 years old and holds the International Master (IM) as well as Woman Grand Master (WGM) titles. Her brother, Borna, plays for the English Federation and holds the FIDE Master title.

A fire broke out at the Trinity Cathedral in St. Petersburg, causing the main dome and another dome to collapse, early in the evening of August 25.

Firefighters fought the blaze, and a helicopter was pressed into service. Staff, emergency service workers and passers by rescued treasured items from the cathedral.

Four hours later, St. Petersburg emergency department spokeswoman Lyudmila Rubasova said the fires were contained but the remaining domes were damaged. Later a departmental spokesmen said that the fire was extinguished.

“The main dome has been destroyed but we could not stop the fire because it was so high and now other domes are in flames,” said a spokesman for Russia’s emergencies ministry.

The cause of the fire was not immediately known, but acting St. Petersburg emergency department chief Leonid Belyayev said the blaze seems to have started on scaffolding on the outside of the church, which was undergoing restoration.

He also said that the most valuable icons and other items had been saved, and that structural damage beneath the roof area was minor.

There are no reported injuries.

Historic gym the site of Benet Academy, Illinois victory over Oswego

Monday, January 18, 2010

Benet Academy Redwings 40 34 Oswego Panthers

The Benet Academy Redwings boys basketball team defeated the Oswego High School Panthers 40–34 Saturday night at Benet’s historic Alumni Gym.

Located at Lisle, Illinois, United States, the gymnasium is home to Benet’s winning streak of 102 consecutive home games. The statewide record lasted from November 26, 1975 until January 24, 1987, when Naperville North High School defeated the Redwings 47–46. Benet also achieved 96 consecutive victories in the Western Suburban Catholic Conference at that time.

The school continued to use the facility, colloquially referred to as the “Old Gym” or the “Small Gym”, until the end of the 1994 season, when a newer athletic center was built on campus. It wasn’t until the late 1990’s when then-coach Marty Gaughan decided to play one game per year in the Alumni Gym to remind the school of its history. “There is just an electricity, and you feel it when you are in there,” said Gaughan, who coached the team from the 1989–1990 season to the 2007–2008 season.

This tradition continued until the 2006–2007 season, when the Redwings played against long-time rival St. Francis High School. Renovations prevented the gym from being used for athletic events over the past two seasons. This game was Benet coach Gene Heidkamp’s first opportunity to coach in the older gym. “So much history and winning has taken place there, which makes it so special. It is something the entire school community is excited about,” said Heidkamp.

Saturday’s game was played at the Alumni Gym at the request of Oswego assistant coach Jim Bagley. He wanted his son, senior forward Chris Bagley, to experience the same atmosphere he had as he played for Benet in the 1978–1979 season. Greg Kwiatkowski was also Jim Bagley’s teammate in Benet, and his son, Joe Kwiatkowski, was in the Panthers’ starting lineup as well. “Two of us who played at Benet together and now our son’s play on [Oswego] together and for them to have a chance to play where we played is going to be a great night and great experience,” said the senior Bagley.

Alumni Gym apparently still had its charm in the second half for the Redwings, whose 15–17 score at halftime worsened into an 11-point deficit in the third quarter. Benet fell behind with a score of 21–29 at the start of the fourth quarter. From that point, the Redwings’ defense began to kick in. Oswego could make only 2 of its 13 field goals in the third quarter. Benet’s offense also gained momentum as center Frank Kaminsky scored all of his 9 points in the fourth quarter. A shot by senior Mike Runger brought the score to 31–31, and two consecutive driving layups by David Sobolewski gave Benet the lead that would last for the rest of the game.

“We hit a dry spell there, but let’s give credit where credit is due. Defense wins games and they are the best defensive team we’ve faced all year,” said Oswego coach Kevin Schnable.

Benet’s Matt Parisi led his team with 15 points, and Sobolewski scored 12. Oswego’s Andrew Ziemnik also scored 12.
